Complete a New Client Form so we can get to know you and your pet a little better.
Our form is online! There is no need to print it and bring it in. New Clients can simply fill out the form online, hit submit, and it comes directly to us. Easy Peasy!
Please share any medical records you have with us so we have the most complete picture of your pet’s health status and previous care. For instance, without current medical records, your new veterinarian may recommend a vaccine your pet is up-to-date on. You have the option to bring physical copies of the records or contact your previous veterinarian and ask the records be sent to us.
We kindly request all previous medical records be provided at least 24 hours prior to your appointment. We are happy to call your previous veterinarian to request them for you! If you already have a copy of your patients records, you can simply take a picture of them with your phone and send them to our email at animalhospitalofdunedin@gmail.com.
