If you are new to Animal Hospital of Dunedin, please fill out this New Client Registration Form it will be e-mailed directly to our office.  Please remember to bring any previous medical history with you when you come for your scheduled appointment.  Don't forget that you will receive a full physical exam and one vaccination (if needed) for only $1.00.  If your pets doctor finds that there other services necessary he/she will be sure to discuss with you a written treatment plan and cost.  Our staff is eager to meet both you and your pet and look forward to providing you with excellent service.
