If you are new to Animal Hospital of Dunedin, please print and fill out this New Client Registration Form. Please bring the completed form as well as any previous medical history with you when you come for your scheduled appointment.  Don't forget that you will receive a full physical exam and one vaccination (if needed) for only $1.00.  If your pets doctor finds that there other services necessary he/she will be sure to discuss with you a written treatment plan and cost.  Our staff is eager to meet both you and your pet and look forward to providing you with excellent service.
